Killed By/Healed By in the Manga: Sailor Moon [with Tuxedo Kamen]
How They Were Killed/Healed: Sailor Moon and Tuxedo Kamen combine their love and their power to connect the Moon Stick with the Maraboshi no Ginzuishou. The power of the Ginzuishou defeats and kills [more or less] this aspect of Chaos.

Killed By/Healed By in the Anime: Sailormoon (Princess Serenity) with the ghosts of the inner senshi to help lend her strength.
How They Were Killed/Healed: She merges with Queen Beryl in a one-on-one showdown with Sailormoon (as Princess serenity). Princess Serenity combines the power of the Ginzuishou with the Moon Stick to create a massive attack, killing both Beryl and Metallia.

Killed By/Healed By in PGSM: Sailormoon, with the help of Mamoru's ghost.
How They Were Killed/Healed: Mamoru absorbs Metallia with the intention of killing himself before she takes over his body, but is too late. A battle ensues between the senshi and Mamoru-as-Metallia in which the senshi are all flattened. Sailormoon takes Endymion's sword in her hands and hears Mamoru tell her to slay him- and Sailormoon obeys him, running the blade through his body.

History:
The First Sailormoon Series (for convenience sake)
Metallia is introduced as the head of the whole operation. Beryl bows to her [which is some indication of her power] and though Beryl secretly plots to one-up Metallia and rule alone, she at least fronts as though she wants what Metallia does. Beryl is the one that stumbles into Metallia who she uses as a puppet for her own plans- to rule the Earth by devouring it whole, plunging it into darkness and chaos. Unlike the anime, Beryl and Metallia are separate entities the entire time. Even after Sailor Venus kills Queen Beryl, Queen Metallia is still around to be contended with. Eventually, of course, Sailormoon kills her off using the Ginzuishou.
Anime:
Metallia is similar in the anime, but she seems more dependent on Beryl for things. She also merges with Beryl when Beryl pleads for her help rather than ignoring Beryl for her own plans.
PGSM:
Until Act 16, they are trying to revive Queen Metallia. It isn't until the end of this act that her name is even mentioned (up until then they refer to it as a 'great evil' they are trying to revive). Metallia is simplified in personality. She is very simply destructive evil, and she only wants to destroy the Earth. The only real twist in this is that her powers are directly linked to the ginzuishou, so each time Sailormoon uses its powers, Metallia becomes more powerful in turn and vice versa when Metallia uses a great amount of power.

NA Dub:
She is devoid of personality in the dub because they change her from a talking Queen to a glorified ball of energy that can be tapped into. In the Dub, Beryl is in charge rather than the Negaforce.
PGSM:
Metallia is certainly simplified in persona... her only goal is to destroy the earth. To do this she creates Youma spontaneously from the ground and sets them loose and she also tries to unleash volatile energy upon the world. She really has no other desire, means or ends.
Why They Were So Evil!:
It was simply in her nature as an aspect of Chaos to be evil.
